What the grant funds

This NOFO will support organizations to strengthen national chronic disease education and awareness for chronic diseases that are not supported through disease-specific CDC funding by implementing activities that enhance surveillance, address priority evidence gaps, develop and disseminate evidence-based educational resources, strengthen strategic partnerships, and promote the adoption of evidence-based prevention, screening, referral, treatment, disease management, and self-management practices. Recipients will review and disseminate surveillance data and evidence-based strategies, develop and/or refine survey tools to improve assessment and surveillance, create and disseminate educational resources for public health professionals, health care professionals, and the public, and strengthen partnerships that expand the reach and use of evidence-based information. This NOFO is expected to improve public health by increasing access to evidence-based education and awareness resources, strengthening surveillance and dissemination of public health information, improving awareness and use of evidence-based practices among public health professionals, health care professionals, and the public, and supporting improved health outcomes for people living with chronic diseases.
